Pre-Employment Resident Requirements

Residents who have been accepted into the program will need to meet the following minimum requirements prior to the start of the program. Note that some requirements are part of (and costs covered by) employer's onboarding process,

Required Items

  • *Proof of full or temporary Utah Physical Therapy license
  • *Proof of current individual professional liability insurance
  • *Proof of membership in: 1) American Physical Therapy Association; 2) APTA Orthopedic section; and 3) Utah Physical Therapy Association
  • Compliance with all University of Utah, University of Utah Health, University Orthopaedic Center, and Salt Lake City VA Medical Center new employee policies and procedures and completion of related orientations and training.Completion of a background check, current within the last 12 months
  • Completion of a clean drug screen, current within the last 12 months
  • Proof of immunizations
  • Proof of current TB test results
  • Proof of current BLS certification

(*not required for VA employment, but required for participation in full residency program)
