How to Apply

Applicants applying to the Doctor of Physical Therapy (DPT) program are required to complete the PTCAS Application. The DPT Program offers three educational pathways: two residential pathways in Salt Lake City and St. George, Utah and one Hybrid pathway.  Each requires its own application to be completed in PTCAS. Application requirements are identical for all three. Applicants may apply to more than one pathway.

THE STATUS OF YOUR APPLICATION MUST BE MARKED COMPLETE BY THE OCTOBER 1 DEADLINE EACH YEAR. A complete application in PTCAS is a submitted application, fees paid, and receipt of all university transcripts. Please be aware that one set of transcripts from EACH college that you have attended or are currently attending, including concurrent enrollment during high school, EVEN IF THE COURSE IS NOT A PREREQUISITE, must be submitted.  Your PTCAS application must be marked complete by Midnight EST (10:00 PM MST). Transcripts must be official and mailed to PTCAS from your registrars' office. We recommend you start this process early. If you pay for Professional Transcript Entry (PTE), you must submit your application no later than September 15 to ensure PTE completion by our application deadline of October 1. No applications will be accepted past the October 1 deadline.

Application Instructions

    PTCAS Application 

    PTCAS allows PT applicants to use a single Web-based application to apply to multiple PT programs. PTCAS is a service of the American Physical Therapy Association (APTA) and is administered by Liaison International (LI), an education information technology company located in Watertown, Massachusetts.

    Your PTCAS application must be marked complete by Midnight EST (10:00 PM MST). Transcripts must be official and mailed to PTCAS from your registrars' office. We recommend you start this process early. If you pay for Professional Transcript Entry (PTE), you must submit no later than September 15 to ensure PTE completion by our application deadline of October 1.

    Definitions of application status can be found in the FAQ portal of the PTCAS website. No application will be accepted past the October deadline. No reminder notices will be sent.

    University of Utah Graduate Admissions Application (Supplemental) Application 

    Only applicants who are offered admission will be required to submit an application to the University of Utah Graduate School using SLATE. The deadline for this application will be sent with the initial admission offer. Transcripts do need to be submitted with the University of Utah Graduate School application, but applicants may submit unofficial transcripts if they have courses still in progress. Upload unofficial transcripts by scanning a paper copy of your transcripts onto your personal computer, or if your institution allows, you can download transcripts electronically and save them as one document onto your personal computer. This applies to ALL applicants, including those who attend the University of Utah as undergrads. Do NOT attach a DARS report nor a link to an electronic transcript, as the Graduate Admissions Office will NOT consider either.

    You do not need to complete the following sections:

    • Resume
    • Letter of Recommendation
    • Statement of Purpose
    • Post Sumission Sections
    • Or, send GRE scores with the University of Utah Supplemental Application.  

    The DPT program begins the second week of May. Indicate on the application that you are applying for Summer Term (May)

    If you are reapplying, when submitting the University of Utah Supplemental application only, the recommendation is to use a DIFFERENT email address than used during your previous application.
