ESSF Off-Campus Permission Code Request Form
*If you are requesting a permission code for an off-campus course, please check the orientation website and only submit a permission code request if you cannot attend the orientation.
This permission request form is for the Exercise Sport Science Fitness (ESSF) Off Campus classes ONLY. ESSF course instructors send out permission codes for on-site classes; you must reach out to the instructor of that course for a permission code request.
Make sure your information below is correct and complete before submitting this request. Please allow 24-48 hours for a response after submitting your request. Your patience is appreciated!
We will only send a permission code to a U-mail address.
